Persistent Systems Unveils AI-Powered Population Health Mgmt Solution in Collaboration with Microsoft

Pune-based Persistent Systems, a global leader in Digital Engineering and Enterprise Modernization, has introduced a Generative AI-powered Population Health Management (PHM) Solution for global market in collaboration with Microsoft.

This solution, aligned with value-based care models, leverages predictive risk modeling and identifies Social Determinants of Health (SDoH) to predict the cost of care better and address patients' non-clinical needs.

The initiative aims to enhance patient outcomes, optimize healthcare provider capacity, and improve cost-effectiveness.

Predictive Insights for Enhanced Patient Care

The PHM Solution aligns with value-based care models, emphasizing the importance of identifying and addressing the causes of negative healthcare outcomes.

Moreover, predictive risk modeling plays a crucial role in identifying patients who may require significant medical assistance, allowing for proactive and targeted interventions. The goal is to enhance patient care by providing timely and intelligent recommendations while optimizing the allocation of healthcare resources.

Regarding the AI software, David Rhew, MD, global CMO & VP, Healthcare, Microsoft, said, "The goal of population health management is to improve the overall health of a population by applying cost-efficient interventions designed to promote better health for groups of individuals.’’

He noted that to fulfill the objective, there is a need to harness data to identify individual needs, monitor the success of interventions, and benchmark against comparative data.

Utilizing Microsoft Azure OpenAI Service & Dynamics 365 Customer Insights

Persistent's newly launched PHM Solution leverages Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights to extract crucial SDoH data from EHR.

Unlike many existing PHM solutions that rely on third-party data sources or survey data, Persistent's approach extracts information from unstructured clinical notes within EHR, providing higher accuracy, improved scale, and increased compliance.

One of the key aspects of the PHM Solution is its ability to assist physicians and caregivers in recommending personalized interventions. These interventions may include community-based programs and care management teams aiming to prevent emergency department visits, avoidable readmissions, and extended hospital stays.

The PHM Solution, developed by Persistent Systems in collaboration with Microsoft, is currently available on Azure Marketplace. This availability ensures accessibility for healthcare organizations looking to implement advanced population health management capabilities.

The solution's focus on predictive risk modeling, extracting SDoH from EHR data, and providing personalized interventions demonstrates a commitment to advancing patient outcomes, optimizing healthcare resources, and contributing to the broader goals of population health management.

In another development, Microsoft unveiled healthcare-focused functionalities for its Fabric data and analytics platform. Initially announced in May last year, this platform acts as a central hub for integrating data from various sources such as electronic health records, medical images, lab systems, medical devices, and claims systems.
